When it is possible

State health insurance is arranged by those who hold residence, permanent residence or citizenship. Without one of those statuses the card is not issued. It is not compulsory, but the logic is simple: if you pay taxes in Serbia, health insurance contributions are part of them, and it is pointless not to use what you already pay for.

How it is arranged

There are three different situations. If you work for a Serbian company, the employer pays the contributions and files the documents with the pension fund — you cannot file the application yourself, but you can request the card's issuance. If you are registered as a sole trader or work in the freelancer regime, you pay the contributions yourself and the insurance renews automatically with your payments. If your residence is based on property, talent or study, insurance can be arranged voluntarily with a separate payment: per current 2026 data that is 5,283.60 dinars a month for the family, even if you are alone; if family members are registered not through reunification but each as a principal applicant, then 5,283.60 per person.

An important practical bonus

Many people do not know that this insurance can be attached to a residence application instead of a private policy. Even the temporary paper issued while the card is being made is accepted. For those who do not want to buy a policy purely to tick a box, this is a good option — but it works only if there is a ground for the insurance.

How to get to a doctor

No one attaches you to a clinic automatically. You simply go to any state clinic at your place of residence, tell the information desk that you want to register, and they make you a card and tell you who your doctor is. From there: appointments with your chosen doctor, referrals to specialists, tests. To register a child with the children's clinic you need a court translation of the vaccination certificate from the country you came from — worth preparing in advance.

Medicines and details that are not advertised

Subsidised prescriptions apply under this insurance, and some medicines cost noticeably less — sometimes a token amount. Doctor's appointments can be booked online. The insurance provides a certificate that is accepted when filing documents too. Two practical points to keep in mind: do not pay the card issuance slip in advance — if something changes, the money is not refunded; and the fee for a newborn's first card does not have to be paid, it is free, as is confirmed locally.

Renewal and when insurance stops working

If contributions and taxes are paid on time, renewal is automatic and no visit is needed. But as soon as payments stop — you froze the business, quit your job, fell into arrears — the insurance stops. It works again once you resume paying. Worth checking if you plan a break from work: the card can slip into a non-working state unnoticed.

What to do step by step

Check whether you have a status that gives the right to insurance. Establish who pays the contributions: the employer, you as a sole trader, or you voluntarily. If residence is based on property or talent, decide what pays off better: paying for insurance separately or buying a private policy. Arrange it and wait for the card, without paying the fee in advance. Register with the clinic at your place of residence, and register your child with the vaccination certificate translation. And watch the payments so the insurance does not lapse unnoticed.
